Basic Membership 

     Our basic membership is an "a la cart" plan designed for individuals who would like to pay for services as needed

$69 a month

(10% yearly discount applied if paid in full)

  Includes:

  * Initial Visit Evaluation

  * Annual Physical with Labs

A la cart services:

At-Home Visits - $149 per visit

Telehealth Appointment - $60 per visit

PREMIUM Membership

    Our premium membership is a comprehensive plan designed for individuals who require additional attention and have chosen to take control of their health 

​

Individual - $279 a month

Couples -   $399 a month

Family Members - $199 a month

 (10% yearly discount applied if paid in full). 

Includes:

* In-Person Visits (House calls) 

* Live Telehealth Visits

* Bi-Annual Labs

* Monthly Vitamin B12 Shots

* Annual Influenza Shots

* 10% Discounts on our MedSpa Products and Procedures

At our clinic, You & I Primary Care, Aesthetics and Wellness, Primary Care Services are performed by our nurse practitioner Dr.Leonard Einstein, DNP,  APRN, NP-C, and our physician Dr. Julio Manuel De Pena Batista MD, who are highly qualified and with a high rate of satisfied patients.
